Initial Comments:
An off-site, state licensure follow-up survey conducted on June 16, July 5, July 6 and July 8, 2022 found that Golden Years Direct Home Care. LLC had corrected the deficiencies cited under Title 28 Health and Safety Part IV, Health Facilities, Subpart H. Chapter 611 Home Care Agencies. The deficiencies were cited as a result of a state re-licensure survey completed on May 2, 2022.
